Disability Care Specialist

As a Support Worker, you will be responsible for providing person-centred care to adults with mental health needs. This includes:
- Assessing the individual's needs and developing a tailored care plan
- Delivering care and support services, including daily living activities and emotional support
- Collaborating with colleagues to ensure seamless delivery of care
- Monitoring and evaluating the effectiveness of care plans
Requirements:
To succeed in this role, you will need to possess:
- A minimum of 12 months' experience in a caring environment
- Experience or specialising in Self Harm, Mental Health, Learning Disabilities, and Autism
- Positive outlook and excellent communication skills
- Ability to work days, evenings, weekends, and Bank Holidays
- Confidence to work alone and as part of a team
