Comprehensive Diabetes Care Specialist

West Leeds Primary Care Network is seeking a highly skilled Comprehensive Diabetes Care Specialist to join our multidisciplinary team. As a key member of our clinical team, you will be responsible for the assessment, planning, and implementation of clinical management plans for patients with diabetes.

About the Role

This role involves working closely with general practitioners, pharmacists, and other healthcare professionals to deliver high-quality care to patients with diabetes. You will be responsible for providing education and training to healthcare professionals across the pathway and developing evidence-based clinical advice in the out-of-hospital system.

The successful candidate will have experience in GLP1 & insulin initiation, independent prescribing, and a strong understanding of local and national policies related to diabetes management. A car driver's license and access to own car are essential for this role.

Requirements
  • Registered Nurse with active NMC registration
  • Independent Prescriber with experience in GLP1 & insulin initiation
  • A Car Driver and access to own car

We offer a competitive salary of £52,809 per annum, depending on qualifications and experience. This role is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975, and a Disclosure and Barring Service check will be required.
