Holistic Needs Care Coordinator

We are seeking a skilled Comprehensive Cancer Care Specialist to join our team at Whittington Hospital NHS Trust. As a Macmillan Holistic Needs Assessor, you will play a vital role in ensuring that patients receive high-quality, patient-centered care throughout their cancer treatment journey.
Key Responsibilities:- Accurate Documentation: Ensure comprehensive and accurate documentation in all patients' notes, including the trust's Electronic Patient Record (EPR) and Somerset Cancer database system.
- Interdisciplinary Collaboration: Liaise with and refer patients to allied health professionals and services as required, promoting seamless coordination of care.
- Patient Advocacy: Act as patients' advocate and assist them in understanding their disease and treatment options, empowering them to make informed decisions.
- Treatment Pathway Coordination: Coordinate patient treatment pathways between departments in primary and secondary care to facilitate continuity of care.
- MDT Participation: Participate in the Breast Multidisciplinary Team (MDT) and attend palliative care and site-specific MDTs as deemed appropriate, ensuring comprehensive care planning.
- Networking and Partnerships: Establish links and develop networking opportunities with Breast CNSs at other Trusts, promoting best practices and collaboration.
- Referral and Communication: Ensure seamless referral of patients to other healthcare professionals and services, maintaining close working relationships with Hospital and Community Macmillan Palliative care teams.
