### Job Title: Triage Practitioner

### Job Summary

We are seeking a skilled Triage Practitioner to join our team at Capture HealthCare Ltd. As a Triage Practitioner, you will be responsible for assessing patients with complex presentations and multi-pathologies, making use of clinical reasoning skills to evaluate patient progress and write reports and assessments.

### Key Responsibilities

* Be professionally and legally accountable for all aspects of your own and delegated work.
* Work within codes of practice and professional guidelines.
* Undertake specialist holistic rehab assessment, including face-to-face assessment or telephone assessment/triage.
* Control and manage triage calls effectively using skilled questioning, active listening, and excellent verbal communication skills.
* Evaluate patient progress, write reports and assessments, and be responsible for the handover.
* Effectively communicate with patients, family members, carers, and colleagues using tact and persuasive skills.

This is an exciting opportunity to work with a state-of-the-art 48-bedded rehab centre offering four different client pathways.
