Job Description: We are looking for a highly motivated and empathetic Support Worker to join our team in Tunbridge Wells. As a Support Worker, you will work closely with adults with physical, mental, and behavioral needs, providing personalized support to enable them to reach their full potential.
About the Role: The successful candidate will be responsible for:
- Developing and implementing individualized support plans in collaboration with colleagues and managers
- Facilitating participation in community activities, promoting social connections and empowerment
- Assisting with daily living tasks, such as personal care, meal preparation, and household management
- Maintaining accurate records and communicating effectively with colleagues and managers
