Job Summary
Live Connected is seeking a dedicated and compassionate Care Professional to join our team. As a Support Specialist, you will play a vital role in providing high-quality care to adults with mental health needs, learning disabilities, dementia, and complex needs.
Key Responsibilities
- Undertake daily living tasks as agreed with the service user, their family, and the professionals involved with the individual service plan.
- Work within the policies and procedures of Connected Living, ensuring the highest standards of care and safety.
- Complete Health and Safety checks within the service, adhering to the policy and procedures in place.
- Lone work confidently, in line with the lone worker risk assessment.
- Ensure safe medication is delivered and ordered to the service users' homes.
- Provide service users with opportunities to express their preference as to the way tasks are carried out.
- Maintain and promote choice and independence to the service users we work with.
- Manage service users' behaviors in the least restrictive way possible.
- Complete daily notes, handovers, and relevant documentation by the end of shift, providing high-quality efficient handovers to the oncoming shift.
- Work as part of a team and wider MDT to support the clients' emotional and physical wellbeing.
- Develop and maintain professional working relationships with service users and the relevant multi-disciplinary team to ensure positive outcomes for the service user.
Requirements
- 6 months experience within a Supported Living Service.
