We are seeking a skilled and compassionate Case Manager to join our team at Social Return Case Management. As a Case Manager, you will work closely with clients who have experienced life-changing injuries, such as brain injury, spinal cord injury, and complex orthopedic injuries.
Key Responsibilities:- Provide comprehensive case management services to clients with complex conditions
- Work collaboratively with clients, families, and healthcare professionals to develop and implement personalized care plans
- Coordinate and facilitate access to medical, social, and community resources to support clients' needs
- Monitor and evaluate the effectiveness of care plans and make adjustments as needed
- Registered nurse, social worker, occupational therapist, physiotherapist, or speech and language therapist with experience working with clients with complex injuries
- Strong communication and interpersonal skills, with the ability to work effectively with clients, families, and healthcare professionals
- Ability to work independently and as part of a team, with a high level of organization and time management skills
