Proactive Care Coordinator

**Job Summary:** We are an innovative, friendly, and supportive team seeking a Proactive Care Coordinator to join our local Community Nursing Services. The successful candidate will work collaboratively with the Community Matron Team to deliver proactive patient-centered care across Guildford and Waverley.
This role is pivotal in ensuring patients receive appropriate care and treatment as close to home as possible. The post holder will work closely with acute and community hospitals to reduce unnecessary admissions and facilitate timely discharges.
The estimated salary for this position is £55,000 per annum, depending on experience.
Responsibilities:- Work as part of the wider multidisciplinary team (MDT) including GPs, adult social care, mental health, community nursing, and geriatricians.
- Collaborate with patients, carers, and families to develop proactive care plans that reduce risks and promote self-management.
- Deliver new and improved pathways for individuals with long-term conditions and complex needs.
Requirements:
- Registered nurse with current NMC registration.
- Recent experience in Primary or Community care.
- Broad range of clinical nursing skills including venepuncture, catheterization, injections, and wound care.
