Clinical Lead

We are seeking an experienced and skilled Community Registered Nurse to lead our Community Nursing Localities team. As a Clinical Lead, you will be responsible for managing and coordinating a team of nurses to provide high-quality, person-centred care to patients with multiple complex long-term conditions.
The team is aligned with the five Portsmouth Primary Care Networks, working closely with a Specialist Diabetes team and Enhanced Care Home Team. You will work within an integrated team to case manage, coordinate, and support patients, using your autonomous advanced clinical skills to plan care interventions and coordinate the team to ensure timely and safe care.
Main Responsibilities- Diagnose, prescribe, and implement care interventions for patients on your caseload, with an emphasis on avoiding unnecessary hospital admissions.
- Plan care interventions for patients with multiple, complex long-term conditions and coordinate the team to ensure patients receive timely and safe care.
- Manage and coordinate the nursing team, ensuring all health and social care needs are met to improve health outcomes, functional status, and quality of life.
- Work collaboratively with other services to ensure patients are appropriately stepped up and stepped down from the community matron caseload.
