About the Job

Thera Trust is seeking a highly motivated and empathetic Care Coordinator to join our team. As a Care Coordinator, you will be responsible for providing person-centred support to adults with a learning disability, helping them achieve their goals and aspirations.

You will work closely with our service users, their families, and other professionals to develop and implement tailored care plans that meet their individual needs. Your expertise and dedication will empower our service users to live independently and make informed decisions about their lives.

Salary: £12 per hour
