Job Summary

Lakenham Surgery is seeking a highly skilled Clinical Pharmacist to join our practice team. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality patient care and contributing to the success of our Primary Care Network.

Main Responsibilities
  • Collaborate with healthcare professionals to support the delivery of the Primary Care Network (PCN) Directly Enhanced Service (DES) and high-quality patient services.
  • Implement Prescribing Quality Scheme initiatives and improve prescribing practices within the practice and across the PCN.
  • Conduct structured medication reviews to facilitate proactive management of patients with complex polypharmacy.
  • Provide clinical medicines expertise to patients and practice teams.
  • Improve patient services and quality through review and clinical audits.
  • Work with the Prescribing Support Team to implement improved processes for prescription management and patient care.
  • Liaise with community and hospital colleagues to resolve medication-related issues.

Job Requirements
  • Advanced assessment and history-taking skills.
  • Ability to apply clinical knowledge in various practice settings.
  • Experience in clinical pharmacy.
  • Good critical appraisal skills.
  • Experience in patient consultations.
  • Good understanding of primary care structure and General Practice.
  • Good clinical knowledge.
Qualifications
  • Registered with the General Pharmaceutical Council.
  • Non-medical prescriber registration or willingness to undertake Prescriber training.
  • Evidence of Continuing Professional Development.
