Job Title: Clinical Pharmacist

We are seeking an experienced and motivated Clinical Pharmacist to join our team at Norfolk Primary Care C.I.C. in Norwich. As a Clinical Pharmacist, you will play a vital role in improving patient health outcomes and the efficiency of our Primary Care team.

About the Role

The successful candidate will work closely with our practice team and other PCN staff to provide and maintain high standards of care for patients. You will be responsible for providing direct, accessible, and timely medicines expertise, including face-to-face contact with patients. Your role will involve optimising medication issues to improve patient care, safety, and supporting clinical staff in the management of patients.

We are looking for a Clinical Pharmacist who is:

  • Experienced in providing medicines expertise to patients and healthcare professionals
  • Able to optimise medication issues to improve patient care and safety
  • Skilled in supporting clinical staff in the management of patients
  • Able to work closely with the practice team and other PCN staff to improve patient outcomes
