Job Title: Health Visitor
Job Summary:
We are seeking a skilled and dedicated Health Visitor to join our team at Wirral Community Health and Care NHS Foundation Trust. As a Health Visitor, you will play a vital role in improving the health and wellbeing of individuals, families, and communities across Cheshire and Merseyside.
Main Responsibilities:
- Identify health needs and raise awareness of health and wellbeing through health visiting contributions to public health.
- Work in partnership with the Local Authority and partner agencies to deliver health care and health promotion services.
- Manage an active caseload and develop and deliver health care and health promotion using evidence-based practice and professional leadership and management.
- Promote positive health to individuals, families, groups, communities, and relevant agencies.
- Contribute to the development of best practice and facilitate shared learning to develop services and initiatives that meet local needs.
Requirements:
- RN (Adult or Child/RSCN) or RM qualification.
- BSc/Public Health Nursing – Health Visiting or equivalent qualification.
- V Nurse Prescriber qualification.
- Leadership Level 6 qualification.
- Mentorship Level 6 qualification.
- Motivational Interviewing trained.
- Solihull Approach trained.
Desirable Criteria:
- Mentorship Level 7 qualification.
- Promotional Interviewing trained.
- Leadership Level 7 qualification.
