About the Role

We are seeking a highly skilled and dedicated Mental Health Practitioner to join our team. As a key member of our multidisciplinary team, you will be responsible for assessing care needs and developing, implementing, and evaluating programs of care within the framework of Effective Care Co-ordination (ECC).

You will work closely with service users, their carers, and other healthcare professionals to ensure that care planning is centered around the individual's needs and preferences. Your expertise will be essential in promoting quicker recovery and ensuring the safe management of medicines.

As a Mental Health Practitioner, you will be expected to work on a rotational shift basis, including weekends and night shifts, to meet the demands of the service. You will be required to be flexible in terms of delivering care within various environments, including community settings.

Key Responsibilities
  • Assess care needs and develop, implement, and evaluate programs of care within the framework of ECC
  • Work closely with service users, their carers, and other healthcare professionals to ensure care planning is centered around individual needs and preferences
  • Ensure the safe management of medicines and adhere to safe practice and principles of administration
  • Participate in ECC and undertake a care-coordinator role as appropriate
  • Assist in the development and implementation of integrated care pathways
Requirements
  • Registered Mental Health Nurse or Dip.COT/SROT
  • Evidence of post-registration study/continuing professional development
  • PSI Training
  • Approved training in phlebotomy
