Vulnerable Adult Advocate - Tripod Partners

Estimated Salary: £30,000 - £38,000 per annum.

About the Role

We are looking for a dedicated Vulnerable Adult Advocate to join our team at Tripod Partners. In this role, you will work closely with our CAD Team to ensure the safety and wellbeing of vulnerable adults in our community.

  • Evaluate referrals sent to our services to identify potential risks and needs.
  • Develop and maintain relationships with professional services to ensure effective collaboration and support.
  • Identify and locate relevant community resources to provide individuals with necessary support.
  • Conduct initial Needs Assessments to determine appropriate housing solutions and support requirements.
  • Carry out home visits where required to assess individual circumstances and provide guidance.
  • Respond to identified risks, taking prompt action to ensure the safety and wellbeing of service users.
  • Support Enquiry Officers in managing calls received that are concerns, referring them to CAD Triage for further assistance.
  • Manage EDT Notifications by carrying out further enquiries or signposting to relevant teams.
  • Review SF1 forms and return them to CAD Triage if they do not meet SG Team criteria.
About You

To be successful in this role, you will need to be a qualified Social Worker with experience in adult social care. You must be passionate about making a positive difference in the lives of vulnerable adults.
