About the Role

Navigo Health and Social Care CIC is seeking a highly motivated and compassionate Mental Health Support Worker to join our team. As a key member of our Acute Older Peoples Mental Health Service, you will play a vital role in delivering individual treatment plans and providing holistic care to our service users.

Key Responsibilities

  • Deliver individual treatment plans and provide holistic care to service users, taking into account their psychosocial and physical healthcare needs.
  • Work closely with Care Co-ordinators and other healthcare professionals to assess, plan, implement, and evaluate individual care plans.
  • Provide regular and practical support to service users and their carers/significant others in developing and managing dignity and independence.
  • Assist with daily living activities, including personal care, and facilitate community and hospital appointments as necessary.
  • Monitor service users' progress, level of functioning, and mental state, and alert the appropriate staff involved in their care.
  • Develop crisis/contingency plans and advanced statements, as per local policy guidelines, in collaboration with the Care Co-ordinator/Key Worker and service user.
  • Maintain accurate records, as required, by existing procedures, and enter relevant details on the service user's case notes.
  • Communicate routine information in an arena where there may be barriers to understanding, and use tact and persuasion in a non-aggressive manner.
  • Provide information on health promotion and wellbeing, and ensure service users understand and have a clear pathway of care across sector/agency boundaries.
  • Attend and participate in agreed training, development, and supervision sessions as an individual and as part of the team.

Person Specification

  • NVQ Level III or Certificate in Community Mental Health, or a willingness to undertake this, or an equivalent level of experience.
  • Experience of working with people with Mental Health problems.
  • An interest in and commitment to on-going personal/professional development.
  • Basic IT knowledge.
  • Flexibility required to meet service needs.
  • A commitment to providing a person-centred care approach.
  • Team Worker.
