Job Summary:

We are seeking a Clinical Case Manager to join our team at Phoenix Centre for Children and Families. The successful candidate will provide evidence-informed therapeutic supports and case management to children/youth and their families.

Key Responsibilities:
  • Provide case management services to children/youth and their families in a clinical setting
  • Develop and implement treatment plans to address mental health concerns, emotional, behavioral, and/or psychiatric symptoms
  • Collaborate with internal and external professionals to provide comprehensive support
  • Maintain accurate records and case manage all files on the caseload

Requirements:

  • BSW or MSW degree from an accredited university program in social work and registered member in good standing with Ontario College of Social Workers and Social Service Workers, or
  • MA or MEd degree from a recognized university program in counselling/psychology/psychotherapy and registered member in good standing with the College of Registered Psychotherapists of Ontario

The estimated salary for this position is $55,000-$65,000 per year, depending on experience.
