Job Title: Dementia Care Manager

We have a fantastic opportunity available as Dementia Care Manager at our luxury care home, Hutton View in Brentwood, Essex. Hutton View is an elegant award-winning 77-bedded Residential, Nursing and Dementia care home offering Relationship-Centred Care, supporting residents at the heart of all we do.

As a Dementia Care Manager, you will lead and manage a committed and loyal caring team on the community, driving all aspects of the Hallmark dementia strategy to ensure your community meets the stringent standards we have created for excellent relationship-centred dementia care.

Key Responsibilities:

  • Oversee the Care Plans for all Dementia residents in the home, ensuring the Care and Nursing teams are aligned and engaged in providing an individualised plan for every resident.
  • Work with the CQC, ensuring relevant documentation and relationships are maintained in line with regulatory requirements.
  • Responsible for ensuring the team providing Dementia support are fully trained and compliant, providing training to the team personally and directing them to changes in policies and practices.
  • Build relationships with other Dementia Care Managers across the 22 Hallmark Luxury Care Homes, identifying common successes and challenges to ensure a consistent way of working.

Requirements:

  • Supervisory or management experience, ideally as a Senior Care Team Lead / Unit Manager or Nurse with a Strong Dementia background.
  • Experience and knowledge of evidence-based practice in dementia care, including in long term care of the older person, including people living with dementia.
  • Qualifications NVQ 4 or 5 / QCF 5 in health and social care.
  • Experience of working with the CQC and aligning to their processes and expectations.
