The Patient Care Facilitator role at George Eliot Hospital offers an exciting opportunity to work in a dynamic and fast-paced environment, providing high-quality administrative support to our Same Day Emergency Care (SDEC) Unit.

About the Role

You will be responsible for ensuring that all patient notes are kept up-to-date and filed accurately, as well as providing an efficient and professional clerical service to our Nursing, Medical, and Surgical Teams. You will also be responsible for communicating with patients, their representatives, and relatives, relaying accurate and applicable information as required.

In addition, you will participate in providing a seamless service that reaches across hospital and community. This will involve tracking case notes in accordance with the Trust Case Note Tracking Policy, and maintaining up-to-date filing in the patient hospital notes in line with the Trust's Medical Records Policy.

Key Responsibilities
  • Ensure that all patient notes are kept up-to-date and filed according to the George Eliot Hospital Medical Records Policy.
  • Provide an efficient and professional clerical service to the Nursing, Medical, and Surgical Teams.
  • Communicate over the telephone and personally with patients, patients' representatives, and relatives, relaying accurate and applicable information as required.
  • Participate in providing a seamless service that reaches across hospital and community.

Salary and Benefits
