We are looking for a highly motivated Adult Social Worker to join our complex needs team. As a Senior Case Manager Learning Disabilities, you will play a key role in reviewing existing care packages to ensure they continue to meet the individual's needs, promoting independence and maximizing their wellbeing.

You will work closely with qualified social care and health care professionals to develop strategies that enable individuals to face life's challenges and live independently and safely in the community. This role requires strong communication and interpersonal skills, with the ability to build effective relationships with service users, families, and colleagues.

Key Responsibilities:

  • Review existing care packages to ensure they continue to meet individual needs
  • Develop strategies to enable individuals to face life's challenges and live independently and safely in the community
  • Work closely with qualified social care and health care professionals to promote independence and maximize wellbeing

Requirements:

  • Current drivers' license and access to a reliable vehicle on a regular basis
  • Ability to travel to various locations within Hampshire and the Isle of Wight or other local authority

Salary Range: £32,149 - £35,241 per annum, pro-rata (dependent on experience)
