Key Responsibilities- Provide dietetic care to Diabetes patients in the outpatient setting, utilizing a combination of face-to-face, video, and telephone clinics, as well as group education sessions.
- Collaborate with the Diabetes Inpatient Care multidisciplinary team to cover inpatient work in the acute hospital setting.
- Develop and implement individualized care plans to support patients with Diabetes, working closely with healthcare professionals to ensure comprehensive care.
- Participate in the development and delivery of education programs for patients and healthcare professionals, promoting best practices in Diabetes management.
- Contribute to the development of the team's CPD program, ensuring ongoing professional development and growth.
