About the Role
We're looking for a skilled and compassionate Care and Support Specialist to join our team in Adult Social Care. As a key member of our locality teams, you'll work closely with NHS colleagues to deliver holistic, integrated support to our community.
As a Care and Support Specialist, you'll conduct assessments, manage complex cases, and develop high-quality care plans that prioritise the needs and voices of service users.
