Job Title: Clinical Frailty Specialist - Older Adults Team
We are seeking an experienced and skilled Clinical Frailty Specialist to join our Older Adults team at Lewisham Hospital, part of Lewisham and Greenwich NHS Trust. This is an exciting opportunity for a registered nurse with advanced clinical assessment skills and experience in frailty specialism to make a real difference in the lives of our patients.
Job Summary:
As a Clinical Frailty Specialist, you will work closely with the interdisciplinary team to provide expert clinical care to older adults presenting to the Emergency Department. You will be responsible for assessing, investigating, treating, and referring patients identified as frail, ensuring high-quality patient care is maintained. Your advanced clinical assessment skills and experience in frailty specialism will enable you to work autonomously, making sound decisions based on your knowledge and experience.
Key Responsibilities:
- Delivery of evidence-based expert clinical care, exercising a high degree of professional autonomy, and demonstrating the ability to make sound decisions based on knowledge and experience.
- Holistic individual consultation of patients using a range of different assessment methods, including medical history taking, physical assessment, clinical investigations, and risk stratification tools, all appropriate to the presentation within the boundaries of the role.
- Initiation of drug therapy/medication within the parameters of agreed clinical guidelines and in accordance with present legislation, supply, and prescribing of medicines.
- Communication with patients in ways that empower them to make informed choices about their health and healthcare.
Requirements:
- Registered Nurse with a minimum of 2 years' experience at Band 7 in an acute NHS setting.
- ACP qualification.
- Experience in Frailty specialism.
- Non-Medical prescriber or working towards qualification.
