Role Overview

This is an exciting opportunity for a qualified Nurse to join our team as a Functional Assessor. The role involves conducting assessments of patients to determine their functional ability and provide recommendations for rehabilitation and support.

The ideal candidate will be a motivated and organized individual with excellent communication skills. You will work closely with our multidisciplinary team to ensure seamless care coordination and delivery.

Key Responsibilities
  • Conduct thorough assessments of patients to determine their functional ability
  • Develop and implement personalized rehabilitation plans to help patients achieve their goals
  • Collaborate with healthcare professionals to ensure comprehensive care coordination
