Primary Care Wellbeing Specialist

We are seeking a skilled and compassionate professional to join our Primary Care Liaison Service (PCLS) team as a Band 6 professional. The successful candidate will work closely with older adults, providing responsive, person-centered, and needs-led mental health assessments and brief interventions.
The ideal candidate will possess excellent frontline assessment skills, a registered Mental Health Nursing qualification, and experience working with older adults. They will be a team player, demonstrating sensitivity, tact, and excellent communication and interpersonal skills.
Main Duties of the JobAs a member of our dedicated team, you will provide telephone triage, video, and face-to-face assessments to service users and their carers across BaNES, working alongside other specialist professionals, including GPs and partnership agencies.
You will play a key role in supporting the continued professional development of the team, promoting a learning culture, and valuing staff wellbeing. This role offers a dynamic and rewarding experience, with an abundance of variety, challenges, and rewards.
