Job Overview
The North Community Mental Health Team in Trafford provides integrated health and social care services, striving to offer quality recovery-based support to individuals with complex and enduring mental health problems.
As a Care Coordinator, you will work under the line management and clinical supervision of the team to maintain and coordinate timely interventions for a caseload of service users living in the community, adhering to the CPA framework and relevant clinical pathways.
Main Responsibilities
You will be responsible for the care coordination of a caseload, assessing service users' mental health and risk, and managing your caseload according to the CPA policy.
This includes respecting the individuality, values, culture, and religious diversity of clients and colleagues, contributing to a service sensitive to these needs.

Person Specification
- Knowledge of service user needs and carers
- Administering depot injections
- Degree or similar RMN qualification
- Experience in care planning
- Working within an MDT and autonomously
