We are seeking a highly skilled and compassionate Care Coordinator to join our Transformation Unit in Manchester. This is an exciting opportunity to work as part of a dynamic team, delivering exceptional care to children with chronic conditions in the comfort of their own homes.

About the Role

As a Care Coordinator, you will be responsible for coordinating the care of children with complex needs, working closely with healthcare professionals, families, and other stakeholders to ensure seamless delivery of care. You will also have the opportunity to develop your skills and gain a Specialist Community Specialist Practitioner qualification.

Key Responsibilities
  • Coordinate care plans for children with chronic conditions
  • Work collaboratively with healthcare professionals and families to deliver evidence-based care
  • Develop and maintain relationships with community partners to ensure smooth care transitions
  • Provide support and guidance to junior staff members
Requirements
  • Registered Children's Nurse with a degree or diploma
  • Teaching qualification and experience of teaching and assessing
  • Awareness of clinical audit and quality assurance initiatives
  • Valid driving license and access to a vehicle
  • Ability to work Monday to Sunday, 08.00am – 10pm, including Bank Holidays
