Job Title: Senior Social Worker

We are seeking a highly skilled and experienced Senior Social Worker to join our Mental Health Integrated Discharge Team in Lancashire County Council.

About the Role

This is an exciting opportunity to work in a fast-paced environment, leading a team of Social Workers in providing high-quality care and support to individuals with mental health needs.

As a Senior Social Worker, you will be responsible for:

  • Leading a team of Social Workers in providing assessment, planning, and intervention services to individuals with mental health needs.
  • Developing and implementing care plans that meet the individual's needs and promote independence.
  • Working collaboratively with other healthcare professionals, including doctors, nurses, and therapists, to ensure seamless care delivery.
  • Providing supervision and guidance to junior Social Workers, promoting their professional development and well-being.
  • Participating in multidisciplinary meetings and contributing to the development of care pathways and policies.
