Key Responsibilities- Collaborate with our multidisciplinary team to provide comprehensive patient care
- Support the delivery of our Directly Enhanced Service (DES) and contribute to the development of new initiatives
- Conduct structured medication reviews to proactively manage patients with complex polypharmacy
- Provide clinical medicines expertise to patients and practice teams, promoting best practice prescribing and medication management
- Improve prescribing practices across the Primary Care Network through clinical audits and quality improvement initiatives
- Interface with community and hospital colleagues to resolve medication-related issues and improve patient care
- Assist with the delivery of enhanced health initiatives in care homes
- Registered with the General Pharmaceutical Council
- Evidence of Continuing Professional Development
- Good critical appraisal skills and experience of patient consultation
- Good understanding of primary care structure and General Practice
- Good clinical knowledge and ability to apply it appropriately in various practice settings
