Leicestershire Partnership NHS Trust is seeking a dynamic Healthcare Collaboration Specialist to join their team. The ideal candidate will have experience in mental health, learning disability, and/or autism, with excellent facilitation and communication skills.

This role offers an exciting opportunity to contribute to the development of a combined health check template and associated resources. As a Healthcare Collaboration Specialist, you will work closely with primary care services and GPs across Leicester, Leicestershire, or Rutland to identify patients for the health check, provide training and support, and evaluate the project's findings.

The successful candidate will be based at County Hall, working 37.5 hours a week, Monday to Friday, with flexibility required for evenings and weekends. This role is a great fit for those passionate about improving health and wellbeing outcomes for people with learning disabilities, autistic individuals, and those with serious mental illness.
