About the Role

Clinical Care Specialist

DHU Healthcare is seeking a skilled Clinical Care Specialist to join our team. As a Clinical Care Specialist, you will be responsible for providing high-quality patient care and working collaboratively with interdisciplinary teams to deliver exceptional healthcare services.

Key Responsibilities

  • Provide autonomous practice and physical examination and assessment skills in primary care settings.
  • Develop and implement care plans that meet the unique needs of patients.
  • Collaborate with healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
  • Stay up-to-date with the latest clinical guidelines and best practices to provide evidence-based care.

Requirements

  • Registered Adult Nurse registered with the Nursing & Midwifery Council (NMC), Paramedic registered with the Health Care Professions Council (HCPC) or Pharmacist registered with the General Pharmaceutical Council (GPhC).
  • Evidence of Autonomous Practice Qualification such as Autonomous practice A33, Level 6 or 7 Clinical Examination Module, Level 6 or 7 Minor Illness Module, RCN Accredited Nurse Practitioner Module, or Emergency Care Practitioner Qualification.
  • Experience in autonomous practice working and multi-professional teams, with the ability to demonstrate a broad and sound knowledge of complex and common conditions in primary care.
