Thisrole involves working with local ethnic and all local communities in Rotherham and Sheffield to raise awareness of the HCD Local Community Model. The model allows individuals with complex care needs to be supported by local people who have been trained and employed to provide care in their own homes.


You will be required to make presentations to local community groups, visit local mosques, social groups, charities, and local specialist health charities. You will also visit individuals who have expressed an interest in being supported in their own home by local people they may already know. The goal is to adopt a local people supporting local people ethos.


The successful candidate will support individuals with more complex needs in staying out of hospitals and long-term care facilities while also helping those who have been placed outside their local area to return home. Additionally, the role aims to provide financial stability for families and communities.



Requirements:
  • Awareness of care or care support at home would be valued.
  • Good communication and interpersonal skills.
  • High level of computer literacy including Microsoft Publisher.
  • You must have a full driving licence and your own vehicle.
  • Be compassionate and caring.
  • Have the confidence and ability to talk to people at a professional level.
  • Be punctual, flexible, and willing to make necessary changes to meet any changing needs of the company.
  • You must have a willingness and the flexibility to travel to various locations.
