Key Responsibilities- Conduct comprehensive assessments of adults and carers to identify their needs and develop personalized care plans.
- Prepare and maintain accurate and clear records of assessments, care plans, and other relevant documentation.
- Work collaboratively with adults, their families, carers, and advocates to establish and maintain strong working relationships.

- Participate in multidisciplinary teams to deliver common objectives and improve the welfare of adults at risk.
- Qualification in social work or a related field.
- Experience of working with adults and carers, including those with complex needs.
- Knowledge of relevant legislation, policies, and procedures, including the Care Act 2014 and Mental Capacity Act.
- Ability to work effectively in a fast-paced environment and prioritize tasks to meet deadlines.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with adults, carers, and colleagues.
