We are seeking an experienced and passionate Community Nursing Specialist to join our team in Surrey Downs Health & Care.

Job Overview

The successful candidate will be responsible for working across our PCN Sites in Surrey Downs, providing high-quality care and support to patients in their own homes and communities.

You will be part of a multidisciplinary team, working closely with GPs, community teams, and other healthcare professionals to deliver patient-centered care and improve health outcomes.

Key Responsibilities
  • To work as part of a close-knit team to provide community nursing services to patients in their own homes and communities
  • To assess patient needs and develop personalized care plans to meet those needs
  • To work collaboratively with GPs, community teams, and other healthcare professionals to ensure seamless care delivery
  • To provide education and training to patients and families on self-care and disease management
