Key Responsibilities
  • Undertake comprehensive assessments of health and social care needs, working collaboratively with the multi-disciplinary team to develop and implement effective care plans.
  • Provide care coordination for a caseload of service users, ensuring their needs are met and they receive timely and appropriate interventions.
  • Work closely with primary care services to ensure seamless transitions and referrals, promoting concordance and adherence to treatment plans.
  • Develop and maintain relationships with community and voluntary agencies, providing access to alternative support and therapeutic services for service users.
  • Contribute to the development and implementation of care pathways, ensuring they are aligned with best practice guidance and person-centered principles.
Person Specification
  • Registered Nurse RMN, Occupational Therapist, or Social Worker with significant experience in a clinical area providing mental and physical health care and assessment.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders, including service users, families, and multidisciplinary teams.
  • Strong understanding of care planning, safeguarding, and advocacy principles, with the ability to apply these in practice.
  • Ability to maintain accurate, timely, and concise clinical records, adhering to Trust policies and procedures.
  • Proficiency in using electronic patient records, Microsoft Word, and other relevant software applications.
