Job Description:

As a Homecare Medicines Optimisation Specialist at The Christie NHS Foundation Trust, you will be responsible for leading the development and implementation of medicines optimisation strategies in the community setting. You will work closely with our multidisciplinary team to identify opportunities for improving patient care, safety, and experience in relation to homecare medicines use and provision.

Key responsibilities include developing and monitoring homecare performance indicators, contributing nationally to the development of homecare medicines optimisation guidance and policy, and managing all pharmacy-related aspects of the homecare services.

You will be a highly motivated individual with a strong background in clinical pharmacy and experience working with homecare services. You will have excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.

  • Develop and monitor homecare performance indicators to ensure compliance and implement action plans for improvement.
  • Contribute nationally to the development of homecare medicines optimisation guidance and policy.
  • Lead the development and implementation of medicines optimisation strategies in the community setting.
