Job Overview:

The NHS is seeking a highly skilled Primary Care Nurse Specialist to join its team. This role will involve providing clinical leadership and developing primary care services within the Greater Middlesbrough Primary Care Network.

Salary: £40,000 - £60,000 per annum.

Responsibilities:
  • To lead clinical care home rounds and coordinate the work of healthcare professionals and non-clinical staff in the care of patients registered at GP practices within the wider PCN population.
  • To work closely with Care Coordinators, GPs, and practice teams to manage a caseload of patients and ensure appropriate support is made available to them and their carers.
  • To participate in weekly care home rounds and multidisciplinary team meetings with Learning Disability EHiCH colleagues in LD Care Homes.
Requirements:
  • Experience of collaboratively working at a senior level with all health, social care, and voluntary sector providers, primary and secondary care professionals, care homes, palliative care services, patients, carers, and commissioners.
  • Experience of clinical audit, evaluation research, and implementation of evidence-based practice.
  • Knowledge of the workings of general practice.
  • A full UK driving licence and vehicle for business use.
