Pharmacist Opportunities in Community Intermediate Care

We are seeking a skilled clinical pharmacist to join our community intermediate care unit, providing rehabilitation and short-term inpatient care for patients. This stimulating role offers the opportunity to develop greater autonomy and responsibility while delivering high-quality clinical pharmacy services.
The successful candidate will work collaboratively with a multidisciplinary team of nurses, doctors, and therapists to provide a comprehensive medicines optimisation service to patients. Excellent clinical knowledge, attention to detail, and good time management skills are essential for this role.
As a registered pharmacist with the General Pharmaceutical Council, you will have completed or be working towards completion of a Postgraduate Diploma in General Clinical Pharmacy. You must demonstrate strong written and verbal communication skills, good organisational abilities, and the capacity to work independently and as part of a team.
This is an exciting opportunity for pharmacists looking to specialise in care of the elderly/intermediate care, with the potential to support mental health services and wider community health services based in Hillingdon.
About the RoleKey responsibilities include:
- Delivering a comprehensive medicines optimisation service to patients and the multidisciplinary treatment team.
- Developing clinical knowledge and skills to practice competently as a foundation pharmacist under the Diploma in General Pharmacy Practice.
- Providing input into policy and procedures affecting the use of medicines at local and Trust-wide levels.
To be considered for this role, you must possess:
- Vocational Master's Degree in Pharmacy.
- Registration with the General Pharmaceutical Council.
- A postgraduate certificate in general Clinical Pharmacy.
Prior experience in hospital pharmacy, particularly in care of the elderly/intermediate care, is highly desirable. Additionally, you should have excellent clinical and critical appraisal skills, teaching and presentation skills, and the ability to communicate effectively with medical, nursing, and other MDT staff.
Estimated Salary: £45,000 - £60,000 per annum (dependent on experience)
