Main Responsibilities:
  • Conduct thorough assessments of service users and carers to identify their needs and develop personalized plans for home environment modifications.
  • Provide expert advice on equipment and home modifications to support individuals with complex needs, ensuring their safety and independence.
  • Collaborate with multidisciplinary teams to develop and implement care plans that minimize the need for care packages.
  • Conduct moving and handling assessments to ensure the safe transfer of individuals with complex needs.
  • Develop and maintain a thorough understanding of safeguarding policies and procedures to ensure the protection of vulnerable individuals.
Requirements:
  • Previous experience working as an Occupational Therapist in a community setting.
  • Manual handling experience and a strong understanding of moving and handling techniques.
  • Registration with the Health and Care Professions Council (HCPC).
  • Enhanced Disclosure and Barring Service (DBS) check.
  • Eligibility to work in the UK.
  • Occupational Therapist qualification.
