About the Role

We are seeking a highly skilled and dedicated Team Leader to join our team at Praxis Care Group. As a Team Leader, you will play a vital role in supporting our individuals with mental ill health, autism, learning disabilities, and dementia. Your primary responsibility will be to lead and manage a team of care staff, ensuring that our individuals receive high-quality care and support.

Key Responsibilities
  • Lead and manage a team of care staff, providing guidance and support to ensure that our individuals receive high-quality care and support.
  • Develop and implement care plans that meet the unique needs of our individuals, working closely with our multidisciplinary team.
  • Ensure that our individuals receive person-centered care, promoting their independence, dignity, and well-being.
  • Monitor and evaluate the quality of care provided, making recommendations for improvement.
  • Collaborate with our multidisciplinary team to ensure that our individuals receive comprehensive care and support.
Requirements

To be successful in this role, you will need:

  • A degree in a relevant field, such as social work, nursing, or psychology.
  • Experience in a leadership role, preferably in a care setting.
  • Excellent communication and interpersonal skills, with the ability to work effectively with individuals, families, and multidisciplinary teams.
  • Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple projects.
  • A commitment to person-centered care and a passion for supporting individuals with mental ill health, autism, learning disabilities, and dementia.
