We are seeking a skilled Recovery Navigator to join our team at The Forward Trust. As a Recovery Navigator, you will play a vital role in supporting individuals with complex needs, including substance misuse and mental health issues, to achieve positive change and reduce reoffending.
Key Responsibilities- Carry a diverse caseload of adult male prison leavers and those on community orders, providing support and guidance to implement their action plan and navigate their recovery journey.
- Deliver comprehensive assessments, create collaborative action plans, and provide structured one-to-one and group work sessions, while maintaining accurate record keeping.
- Work across a cluster of community delivery sites, providing flexible and responsive services to meet the needs of our clients.
- Develop and maintain strong relationships with key partners, including probation staff and treatment services, to ensure effective delivery of outcomes.
- Contribute to the development of subject matter expertise in particular vulnerable groups or needs, providing support to colleagues across the team.
- Knowledge and experience of the criminal justice sector, with a strong understanding of substance misuse and related services.
- Excellent experience of managing high caseloads, carrying out risk assessments, and creating SMART action plans.
- Ability to work effectively with people from diverse backgrounds, using motivational techniques and trauma-informed approaches.
- Strong communication and organizational skills, with the ability to prioritize tasks and meet targets.
- Level 3 substance misuse related qualification or working knowledge of substance misuse services.
