Job Title: Recovery Navigator - D&R

Join The Forward Trust, a leading social enterprise empowering individuals to break cycles of crime and addiction, as a Recovery Navigator in our Dependency and Recovery Services.

About the Role

We are seeking a motivated and self-driven individual to work with our client group, supporting them on their recovery journey. As a Recovery Navigator, you will be part of a hybrid model, working across a cluster of community delivery sites, carrying a diverse caseload of adult male prison leavers and those on community orders within the Probation services.

Key Responsibilities
  • Implement comprehensive assessments, create collaborative action plans, and deliver structured one-to-one and group work sessions.
  • Provide support and continuity, signposting clients to relevant services.
  • Maintain accurate record-keeping throughout.
  • Work across London, providing flexibility and resilience against changing demand.
  • Deliver services enhancing access to treatment and providing additional support and guidance for clients with complex needs.
Requirements
  • Experience of working within substance misuse services.
  • Carrying out comprehensive assessments, risk management plans, and designing SMART care plans.
  • Delivering structured interventions to service users.
  • Facilitating groups and using motivational interviewing techniques.
  • Strong IT skills and understanding of continuity of care, holistic care needs, and community services.
  • Experience promoting and supporting Equality and Diversity.
