Job Overview

An exciting opportunity has arisen for a skilled and dedicated Community Nurse to join our Health Visiting Team in Swansea Bay University Health Board.

Main Responsibilities

The successful applicant will carry out duties delegated by health visitors in a range of settings, including patients' own homes, clinics, and groups. The community nurse will be part of a dynamic health team to provide advice and support to children and their significant others, building resilience and providing a positive framework in the crucial early years.

Key Duties
  • Participate in assessing, planning, delivering, and evaluating care for patients/clients as part of a multi-disciplinary team.
  • Work with children and their significant others at home or in a group environment, carrying out clinical responsibilities.
  • Liaise with and work in partnership with members of the wider Health Visiting Team and voluntary and statutory agencies as appropriate.
  • Contribute effectively to delegated administration and clerical duties.
