Community Health Care Professional

We are seeking an experienced Adult Community/District Nurse to join our team in the West Midlands Area. This is a fantastic opportunity to work as part of a supportive and friendly environment, providing high-quality patient care and supporting the community.
The successful candidate will have access to their own transport, as travel will be required across various locations. We require experience in compression, wound assessment, catheterisation, central lines, palliative care/support, medical/administration, PEG, and phlebotomy.
Key Responsibilities:
- Undertake responsibility for caseloads, assessments, planning, and evaluation of patients' care needs.
- Liaise with all healthcare professionals to ensure effective service delivery.
- Support the district nursing team and contribute to the care of patients, helping them remain independent in their own homes.
Requirements:
- A minimum of one year's experience working as an Adult Community/District Nurse in the UK.
- Valid NMC registration.
- Ability to provide references from seniors/managers covering three years.
What We Offer:
- An excellent hourly rate of up to £787.50 per week, depending on experience.
- 0.45 pence per mile tax-free mileage reimbursement.
- Flexible shift patterns, including 08:00am - 16:00pm or 08:00am - 17:00pm.
