Learning Disabilities Specialist Nurse

We are seeking an experienced and skilled Learning Disabilities Specialist Nurse Assessor to join our team. The successful candidate will be responsible for assessing clients with Learning Disabilities in various settings to determine eligibility for Fully Funded NHS Continuing Healthcare or Funded Nursing Care.
Main Responsibilities:- Conduct comprehensive assessments and reviews of clients with Learning Disabilities to determine eligibility for NHS Continuing Healthcare or Funded Nursing Care.
- Act as a senior nurse assessor and coordinator of a Multidisciplinary Team to establish eligibility and work with clients and their families to identify suitable care packages.
- Ensure the review process of all Funded Nursing Care/Continuing Healthcare is undertaken in accordance with the National Framework Guidance for Continuing Healthcare.
- Form part of the senior nurse assessor team for the CHC team, ensuring clients receive high-quality care and provide specialist advice regarding Learning Disabilities to the CHC team.
- Lead on ensuring all clients with a Learning Disability who have care delivered through a Personal Health Budget have appropriate care and support plans in place.
- Educated to degree level in a relevant subject or equivalent level qualification or significant experience of working at a similar level in a specialist area.
- 1st Level Registered Nurse (Adult or MH) with extensive experience of working with LD clients.
- Member of a relevant professional body.
- Evidence of continued professional development according to NMC.
- Excellent knowledge and practical experience of the Continuing Healthcare Framework.
