Key Responsibilities- Assess and manage patients with complex frailty needs, working closely with multidisciplinary teams to develop and implement holistic care plans.
- Provide expert clinical guidance and support to colleagues, promoting best practice in frailty care and contributing to the development of our Frailty Service.
- Collaborate with community partners and stakeholders to ensure seamless care transitions and effective service delivery.
- Contribute to quality improvement initiatives, using data and evidence to inform practice and drive service development.
- Participate in education and training activities, sharing expertise and knowledge to enhance the skills of colleagues and support the development of our Frailty Service.
- Post-graduate education and experience in a relevant field, with a strong focus on frailty care and older adult health.
