Service Care Solutions - Social Work is seeking a dedicated and experienced Social Worker to join their Learning Disability & Autism Team within Adult Social Care. This role offers the opportunity to work closely with individuals with learning disabilities and autism, ensuring timely assessments and support.

Key Responsibilities:

  • Transition Management: Oversee the transition allocation list, ensuring timely assessments and smooth transitions for individuals entering adult services.
  • Case Management: Manage a caseload of complex cases, providing assessments and managing risks effectively.
  • Timely Interventions: Address any backlog of referrals and ensure that assessments are carried out in a timely and efficient manner.
  • Collaboration: Work closely with other professionals, families, and carers to develop person-centred care plans and ensure holistic support for individuals.
  • Teaching and Support: Support the teaching and assessment of social work students during their placements and offer guidance to less experienced staff members.
  • Court Proceedings: Provide evidence in court as required, particularly in relation to care proceedings.

Requirements:

  • Have substantial experience in adult social care, particularly within learning disabilities and autism services.
  • Hold a degree in Social Work and be registered with Social Work England.
  • Enhanced DBS.
  • Access to own car.
