Leaders in Care Recruitment Ltd is seeking a highly skilled Senior Adult Nurse Case Manager to join their team. This role is perfect for a motivated and experienced RGN looking to further their career and contribute to the growth of a well-established provider of complex care packages.

The ideal candidate will have a supportive management style, up-to-date experience of tracheostomy care and ventilators, and advanced communication skills to liaise effectively with patients, families, and the multidisciplinary team.

The successful candidate will be responsible for managing a reduced case load of service users, supporting the business with growth plans, and mentoring Adult Nurse Case Managers. They will also design and deliver tailored care plans specific to individual clients' needs, effectively manage the multidisciplinary team through strong communication and teamwork, and provide ongoing monitoring, support, and assessment of clients' physical and physiological needs.

The role offers a competitive salary of up to £50,000 per annum, depending on experience, as well as a range of benefits including 33 days' holiday, a private pension scheme, and an excellent learning and development programme.
