Job Summary

We are seeking a highly skilled Clinical Pharmacist to join our Primary Care Network. As a key member of our team, you will be responsible for providing expert medication advice and support to patients, healthcare professionals, and other stakeholders.

Main Responsibilities
  • Provide clinical assessment and treatment of patients, utilizing expert knowledge of medicines and disease areas.
  • Offer specialist expertise in the use of medicines and medicine information, ensuring optimal patient outcomes.
  • Conduct pharmaceutical medication reviews and health education sessions, promoting patient understanding and adherence.
  • Lead on person-centered medicines optimization and quality improvement initiatives, driving excellence in patient care.
  • Support the practice dispensary, ensuring efficient and effective medication management.
  • Act as a key contact for medication-related queries, providing timely and accurate advice.
  • Develop and lead clinics focused on medicines optimization, including anticoagulant monitoring and care planning.
  • Conduct care planning, including mental health, admission avoidance schemes, and end-of-life patient support.
  • Undertake minor ailments triage, directing patients to appropriate care pathways.
  • Capture relevant patient health information, contributing to quality outcomes and performance metrics.
  • Provide clinical advice and expertise on medication, ensuring safe and effective use.
  • Manage repeat prescribing reauthorizations, reviewing requests and medicines reaching review dates.
  • Review prescribing against local and national targets, engaging with the prescribing team to address any issues.
  • Conduct structured medicines optimization reviews in care homes, promoting improved patient outcomes.
  • Contribute to reducing medicine wastage across practices within the PCN.

Person Specification
  • Awareness of common acute and long-term conditions, with the ability to demonstrate general pharmaceutical skills and knowledge.
  • Ability to plan, manage, monitor, advise, and review general care plans for patients with long-term conditions.
  • Accountability for delivering professional expertise and direct service provision as an individual.
  • Ability to recognize priorities when problem-solving and identify deviations from normal patterns, referring to seniors or GPs with confidence.
  • Ability to follow legal, ethical, professional, and organizational policies, procedures, and codes of conduct.
  • Ability to assist patients in decisions about prescribed medicines and support adherence as per NICE guidance.
  • Non-judgmental attitude and ability to be flexible and adaptable.
  • Self-motivated with excellent communication skills.
  • Understanding of the need for patient confidentiality.

Qualifications

  • Completion of a degree in pharmacy and registration with the General Pharmaceutical Council.
  • Post-qualification experience in pharmacy (hospital, primary care, or community), or working towards obtaining this.
  • Qualified independent prescriber, or working towards this.
  • Member of the Royal Pharmaceutical Society (RPS).
