Job Summary

We are seeking a highly skilled and motivated Clinical Pharmacist to join our Primary Care Network. As a key member of our team, you will be responsible for providing expert pharmaceutical care to patients, working closely with GPs and other healthcare professionals to optimise medication use and improve patient outcomes.

Main Responsibilities
  • Conduct clinical assessments and provide treatment plans for patients with specific disease areas.
  • Provide specialist expertise in medicines use and information.
  • Deliver pharmaceutical medication reviews and health education advice to patients.
  • Lead on person-centred medicines optimisation and quality improvement initiatives.
  • Support the practice dispensary and act as a key contact for medication-related queries.
  • Run clinics for anticoagulant monitoring and other medicines-related interventions.
  • Conduct care planning, including mental health, admission avoidance schemes, and end-of-life care.
  • Undertake minor ailments triage and direct patients to appropriate services.
  • Capture patient health information and contribute to quality outcomes frameworks.
  • Provide clinical advice and expertise on medication.
  • Manage repeat prescribing reauthorisation and review prescribing against local and national targets.
  • Carry out structured medicines-optimisation reviews in care homes.
  • Contribute to reducing medicine wastage across the PCN.

Person Specification
  • Awareness of common acute and long-term conditions in general medical practice.
  • Ability to demonstrate general pharmaceutical skills and knowledge in core areas.
  • Ability to plan, manage, monitor, advise, and review general care plans for patients with long-term conditions.
  • Ability to demonstrate accountability for delivering professional expertise and direct service provision.
  • Ability to recognise priorities when problem-solving and identify deviations from normal patterns.
  • Ability to follow legal, ethical, professional, and organisational policies, procedures, and codes of conduct.
  • Ability to assist patients in decisions about prescribed medicines and support adherence.
  • Non-judgemental attitude and ability to be flexible and adaptable.
  • Self-motivated with excellent communication skills.
  • Understanding of the need for patient confidentiality.
